Additional AFC Benefits
...that prevent or delay institutional care, reduce healthcare costs, improve care and quality of life for clients and their caregivers:

  • AFC staff can establish Client-Caregiver matches for potential Clients without a Caregiver.
  • A Client can live in a Caregiver’s home or the Caregiver can move into the Client’s home.
  • AFC Registered Nurses monitor Client medical conditions and treatments, while AFC Social Workers monitor the quality of the placement, provide support and crisis intervention.
  • AFC staff partner with other medical and behavioral care providers (DDS, DMH, physicians, case managers, other agencies) to ensure continuity of care.
  • AFC staff work with Clients and Caregivers to develop an individualized care plan that is modified as Client needs change.
  • Non-family Host Caregivers earn an additional $450 of rent income from Clients.
  • AFC staff provide 8 hours of annual at-home clinical teaching and training, including:

    - Health and medication management
    - CPR/First Aid and good body mechanics
    - Behavioral interventions
    - Communication skills
    - Healthcare proxy
    - Home safety and emergency procedures
    - Caregiver stress management
  • 24-hour emergency coverage available from AFC nurses and social workers.
