Adult Family Care Host Caregiver

Hosts

Adult Family Care (AFC) hosts are single people, couples, and families with an extra room, time to spare and plenty of compassion. Hosts provide a private bedroom, meals, and depending upon the participant's needs, limited personal care, transportation assistance, and medication reminding. Hosts undergo a thorough assessment process to ensure the best possible match with a potential participant. Hosts can have up to three AFC participants in their home.

Host payments

  • Medicaid pays $19.00 per day in TAX-FREE income (approximately $570 per month)
  • Participant pays at least $400 per month in Room and Board
  • Host application process
  • Contact the Adult Family Care program at 617-628-2601
  • We conduct a home visit to interview your preferences regarding potential participants and confirm that the home is up to regulations
  • We obtain references and a criminal history check
  • Once you are approved, we match you with potential participants that fit your preferences and facilitate the "getting to know you" process by three meet and greet sessions, the last of which is an overnight.

AFC Staff

The AFC staff consists of two registered nurses, two social workers, and a program director. Since the AFC program is part of Somerville Cambridge Elder Services, we also have back up nursing and social work available as well as 24-hour beeper coverage.

Pre-Matches

A pre-match is a situation where the potential host and potential participant already know one another. This can include domestic partners, friends, acquaintances, and distant relatives. A pre-match situation reduces the time to solidify the placement because the three "meet and greet" sessions can be waived.
