Adult Family Care: The Caring Choice

Adult Family Care (AFC) is a vital program that helps older people and younger people with disabilities, who need assistance with daily activities, to live independently in a supportive family environment. AFC nurses and social workers support both, Clients and Caregivers. We serve communities in and around the Greater Boston and Merrimack Valley area.
Clients
Benefits
- Live with a trained Caregiver who provides daily care, meal preparation, as well as assistance with personal care and medical needs.
- Are eligible for up to 5 days per week of Adult Day Health.

Eligibility
- Individuals 16+ who are unable to live alone and who have a chronic medical or psychiatric diagnosis requiring assistance with at least one activity of daily living (bathing, dressing, toileting, transferring between positions, ambulating, or eating).
- Individuals must also be MassHealth eligible (Standard, CommonHealth, SCO) or able to pay privately.

Caregivers
Benefits
- Receive a tax-free stipend up to $18,000.
- Enjoy two weeks of annual respite relief with pay.

Eligibility
- Anyone, including a friend or family member of the Client, excluding spouse or a person who is legally responsible for the Client.
- Individuals also need to pass a thorough screening (including phone interviews, home assessments, reference checks, criminal history and OIG Medicaid fraud check) and have a recent physical exam and TB screen.
- All applicants must be able to handle Caregiver responsibilities (including client care, documentation, appointment and transportation coordination).
Note: Host Caregivers can have up to 3 AFC Clients in their home and no more than 2 Level 2 Clients.

Caregivers with expertise in Mental Health support are particularly needed. We are currently looking for caregivers in and around the Greater Boston and Merrimack Valley area.
Information and Referrals
Information needed for client and caregiver referrals:
- Individual and emergency contact information
- Date of last TB testing (within 1 year)
- Date of last PE (within 1 year)
- PCP contact information
- Health insurance information (MassHealth required)
Additional information needed for client referrals:
- Host contact information
- List of diagnoses
- Confirmation of last medical visit within last 90 days
